I am aware that getting a tattoo involves certain risks including but not limited to pain, discomfort, allergic reactions, infections, scarring and other complications. I acknowledge that I have been informed of the risks associated with getting a tattoo and I accept those risks.
I confirm that I am 18 years of age or older and that I am not under the influence of drugs or alcohol at the time of my appointment. I understand that a deposit is required to secure my appointment time and that this deposit is non-refundable.
I acknowledge that the tattoo design may be modified slightly from the original design based on the tattoo artist's professional opinion. I confirm that I have approved the final design before the tattooing process begins.
I agree that the studio does not have a way of identifying if I am allergic to the elements or ingredients that will be used for my tattoo.
I understand that there will be no refunds for the completed tattoos, regardless of how I feel about the tattoo or if an allergic reaction occurs.
I agree to follow the tattoo artists aftercare instructions carefully and to seek medical attention if I experience any adverse reactions or complications.
I indemnify and hold harmless the tattoo shop and tattoo artist against any claims, expenses, damages and liabilities. (Including any injuries in the studio including, but not limited to, slipping, falling, etc.)
I give the artist permission to take photographs and/or videos of my tattoo for use on social media and/or their professional portfolio.
